Transcripts

To ensure accuracy, transcript requests may take up to four weeks to process. Transcripts are processed on a first-come, first-served basis. No rush orders are accepted at this time. Please apply early to allow sufficient time to process your transcript.

To download the "Transcript Request Form" please right click on this link and select "Save Target As..". You will need Adobe Acrobat Reader to view the form. If you don't have Adobe Acrobat Reader, please click on link to download and then install the software. Please fill out the form as completely as possible and fax or mail the request to Admissions Office.

 

Note that transcript requests will not be taken or processed during general registration and add/drop periods.

 

Transcript fees are $5.00 for an official transcript and $2.50 for an official copy issued to the student. The fee may be paid by check or cash if in person. Off-island requests must be paid by money order or certified check. No off-island personal checks will be accepted (Addendum to ASCC General Catalog 2006-2008). Note that most institutions will only accept a transcript as an official transcript if it is sent directly from ASCC to the institution.

 

To request your transcript in person:

1. Pick up transcript request form from the Admissions and Records Office
2. Make the appropriate payment to the Business and Finance Office.
3. Submit the payment receipt and the completed form to the Admissions and Records Office
4. Your request will then be logged in and handled on a first-come, first-served basis.
If a student is unable to apply in person:

1. Fax or mail a request to the Admissions and Records Office giving permission to release your transcript. Please include your signature. The signature is required before processing will begin. No exceptions will be made to this policy.
2. Please state the name of any person authorized to pick up your transcript. If the name is not authorized by you in your letter, then the transcript cannot be released. No exceptions will be made to this policy.
3. Make payment arrangements through the Business and Finance Office.
4. Your request will be logged in and handled on a first-come, first-served basis.

Please make sure to state in your request where you want us to send your transcript to (provide address), and if whether or not you want us to fax it first before putting it in the mail (provide fax number and address), or if you want a relative living on island top pick it up for you, his or her name must be mentioned.
